• Please bring your insurance card with you.

• Please bring a list of your current medications and doses.

• While our practice uses an electronic record system, it may not interface with the laboratory you use or the system used by your other doctors.  Your visit will be most productive if any relevant office notes, records, laboratory, and radiology tests results are available at the time of your visit.  Prior surgery and angiogram reports are valuable. You may have those records faxed to us at 732-981-0413, but it is safest to bring paper copies with you that we will scan into your chart.
